Anatomical or reverse shoulder replacement?

There are two main types:

  • Anatomical total shoulder replacement replaces the ball at the top of the arm bone with a metal ball and the socket with a plastic socket — recreating the shoulder's natural shape.
  • Reverse shoulder replacement swaps the ball and socket around, so different muscles move the arm. It is used when the rotator cuff tendons are torn or no longer work.

Which one you need depends on your rotator cuff and the pattern of damage, and the surgeon decides after examination and imaging. The type can affect the price, so make sure any quote states which replacement it covers.

What recovery involves

Shoulder replacement recovery is slower than people expect, mainly because of the sling. Based on NHS patient information:

When What to expect
First days Usually one or more nights in hospital; physiotherapy starts with the arm in a sling
0–6 weeks Sling worn for up to six weeks, including in bed; gentle assisted exercises; you do most things one-handed
6–12 weeks Exercises progress to moving the arm on its own and strengthening; no heavy lifting
8–12 weeks Typical return to driving, once you no longer need the sling and have good shoulder movement
At least 3 months Avoid lifting weight with the arm — even a kettle, pan or iron
6–12 months Pain and function continue to improve

Return to work is usually at least eight weeks for a light job, longer for heavy or overhead work. If you care for someone, arrange help before the operation: for six weeks you will be managing with one arm.

Travelling with a shoulder in a sling

A sling changes the practicalities of the trip more than the operation itself:

  • Bring a companion. You will not be able to lift a bag, open heavy doors or manage a coat on one side.
  • Book airport assistance when you book the flights, or at least 48 hours before departure.
  • Pack for one hand: front-opening, loose tops; slip-on shoes; a small bag you can carry on the good side.
  • Flying soon after surgery: NHS patient information advises discussing it with your consultant and the airline's medical department if you want to fly within six weeks of the operation, because of the risk of blood clots. See flying after surgery.
